I know little about these tings. Perhaps some of you can help?
- Is an offset of 42mm far from the original offset? (I am allowed to have an offset of +/- 7mm) An offset of ET42 should be fine for the Avensis, provided your wheels are no bigger than 16x7.5J (idealy they would be 16x7J.. the 7J part is the width of the alloy)
- Is there any reason to use spacers? You shouldn't need spacers.. spacers are only needed when the wrong offset causes the wheel to hit the inner wing/strut, they would go on the hub before the wheel and would give you an extra 5 - 10mm of clearance (sometimes longer wheel nuts/bolts would be rquired as you would be loosing 5 -10mm of the threads) i wouldn't recomend fitting wheels that rquire spacers.. if the wheels don't fit properly, they shouldn't be on the car.
- Does it matter what the center bore is as long as it is large enough? If it is too large you will need to fit centralising rings.. these convert the bore hole of the wheel to the same size as the hub (54.1mm)

Ideal wheel size for 16" = 16x7J (7.5J max)
Going larger would be possible with some other modifications.. Ken might be able to offer more help if required.
