2.0 Avensis Vaccum Leak 3S-Fe

I finally managed to stop my engine racing...

Turns out the Manifold gasket hadn't sealed properly, so after a correct tightening and yet another new gasket all is well...

I also took the rocker cover off, in order to get the injectors to seat in their new insulator rings in the head correctly...

Started her up and she idles okay now, albeit a bit lumpy, when it warms up there is a small misfire at idle and it sits eventually at 750rpm...

I can hear a loud sucking noise and if I stick my hand over the throttle body, the car with either stall, or try to stall and control to run (just)

I wonder if I've put the Vacuum pipes on wrong?

Can anyone advise me where the Vacuum Pipes connect at the top of the Engine, on the throttle body, the EGR valve and BVSV thingy...

Many Thanks.
