Toyota Avensis D4D Accumulator Problem

HI all any one had problem with the accumulator i replaced the washer inside the box its easy to open and replace the washer i have done it twice now keep losing the fluid and the washer keep geting damaged leaking after few days

I havve jsut bought new one from mr t cost me £90 what is the problem with it will have the problem with the new one is it the flywheel worn or pressure plate worn causing it to bust milage its done around 180k used as taxi
