Cold Starting

My March 2002 Avensis D4D 125000K miles, does not start from cold readily.I have one glow plug tip seized in the head.Initially the engine turns over but does not start, then on the second try it starts. The fuel filter was changed 5000 mls ago and the system has been cleaned with BG244.Once started the car has no power for the first half mile after which the power is fine all day. The engine revs. reduce to 750 after a mile which it has always done, then after about another half mile they increase to about 1200 until the engine is at near running temp. when the revs. reduce to 750.no further problems for the rest of the day, even after sitting for 5-6 hours. Anybody any suggestions ?

firstly all glow plugs must be in order to proceed further.

Thanks for the info. I was thinking that maybe the coolant sensor was the problem. I have checked and cleaned all the connections to the fuel supply apart from under the rear seat, and I realise that the glow plugs are an intrisic part of the starting system but I am delaying having to have the head taken off to remove the tip, £500 I have been quoted, I have checked the resistance of the remaining plugs and they are functioning. I don't dispute that eventually the glow plug tip will have to removed.

Apart from the glow plugs what other cold start system(s) are there and what sensor(s) govern them. Is the tank fuel pump only operational once the engine is turning over? I disconnected the supply at the filter and turned on the igntion but there was no fuel flow !!
