This pic above is spot on, there where two manufacturers of the lower steering shaft one of which has a fault, Toyota will inspect the bolt hole at the bottom of the shaft if the bolt has a countersunk bolt hole then a new shaft & bolt is fitted, if the shaft has a flush bolt hole then just a new bolt is installed.
