Lockup Torque Convertor

Can anyone explain how the lockup torque convertor functions in an SV21 Camry Auto.

I understand it locks the convertor's input and output rather than driving through fluid. What activates this and does it only lock up in overdrive? If the lockup is faulty, is it possible for it to lockup in all gears?

Think its operated by oil pressure much like the rest of the auto box
