Gear Knob And Rubber Wheel Arch/door Trim?

I just bought my RAV4, the previous lady owner must have worn a lot of rings and the gear knob is scratched where as the rest of the car is mint. Is it screw off? (RAV4 2007 diesel, 6 gear).

Next question. We take our aging greyhound out a lot and she is having trouble getting in and out as she tries to step onto the curve of the wheel arch inside the rear passenger doors (we don't and won't put her in the boot). Is there any rubber self adhesive trim that can be fitted?
