Engine Fumes

Hi everyone

I have a 2011 rav 2.2 diesel. Firstly I have a tapping noise that sounds like it is coming from the back of the engine around the injector pump area. The noise is quite loud at idle and when under load. I thought it may have just been diesel rattle but now not so sure. Also while looking for this noise I happened to remove the oil filler cap with the engine running and it seems to fume a lot from the top of the engine, Is this normal? The engine has only done about 35000 miles and has always been serviced.
