T180 - Misfire and Limp Mode

My T180 has 2 problems. After looking through various forums, it seems that these are common problems on this car.
 
  1. A misfire/hesitation around 2000 r.p.m. Above 2000 r.p.m. the turbo kicks in and car drives OK. Since the hesitation occurs at the point where the turbo kicks in I'm wondering whether there is a turbo related problem. A faulty sensor or leaking hose somewhere. Or even a fault with the turbo itself except that, when it does kick it it goes well.
  2. Car occasionally goes into limp mode with the engine light/TRC/VCS lights on. This usually happens when going down long, steepish hills. In fact there are three local hills where this happens in the same place about 90% of the time. I've checked the fault code when this happens and it always shows "Exhaust gas recirculation flow", even after fitting a new EGR valve. Possible causes that occur to me are: low voltage to the stepper motor, bad connection or broken wire. But why would these cause the fault to occur when going down hill? Could it be fuel related such as clogged fuel filter or dirt in the fuel? Or, could it be caused by the DPF - e.g. back pressure when going down hill? 
 
So I've done the following:
  • New EGR valve.
  • Replaced injectors with refurbished ones. - but not the 5th injector
  • New MAF sensor.
  • DPF cleaned. 
 
Any thoughts on these problems? I'm getting to the point where I don't want to throw much more money at it.

Is the diagram on the turbo moving with ease? Also was it a brand new egr?

P0400 Relates to Exhaust gas flow and indicates a restriction/blockage in the EGR system which is more than just an EGR valve especially on the 180 engines. if the main valve 25620 is clean and moving freely, the intake manifold is clean then there is likely a blockage further up the EGR pipe work, see picture attached there is another valve 25630 however the likely point of blockage is the EGR cooler ( a water/air cooler ) which is buried under the Turbo at the back of the cylinder head

The first 3 things could just be cleaned, no point in just throwing money away. But this looks like good advice and you should change the fuel filter much more on diesels.

Hi Mike, I'm having exact same issue almost word for word with 2008 rav t180, also fitted new egr, fuel filter, fuel treatments. Did you get to the bottom of the issue? Any info would be much appreciated.

Any new advice with this problem?  Exactly the same problem with my 2008 Avensis Diesel Hatchback2.2 D-4D (first occurred  few weeks back). Just had new egr valve fitted, clogged up inlet manifold cleaned out, new oil, filters etc. A lot of money for almost no improvement. Garage suggested likely cause now is either head gasket failure or worst still cracked head. (Their reasoning is there's a excessive build up of pressure in the cooling system being caused by a gas leak)  Either way mega bucks without a guarantee of a fix. Could it be something far simpler? Any help please

I went to buy a Rav4 T180 which had similar problems was from a garage from someone I knew. It turned out to be the fuel injectors was sent to a diesel specialist to resolve. Sorry see you have done the injectors anyway

Hi I have T180 Avenisis 56 reg TRC VSC lights on, the car will not accelerate up hill past 20 mph, it seems like limp mode. Any ideas?

Is this all happing in the 2.2d4d Toyota engines by chance is none of these cars in the comments the 2.0d4d 1adftv engines .just wondering
