HELP! yaris D4D, 2004 mysterious faults

I'm hoping someone can help me with my 2004 Yaris Verso d4d! I am runnig out of ideas.

Problem 1:

The car is very hard to start. Starts at once with the use of a little start gas. But a push start always works. 

 

Problem 2:

This is the worst one. If i push the throttle and pass 3000 rpm the car dies. After waiting a couple of minutes it will start again either with start gas or push start. 

Things that are checked / replaced:

Front fuel filter

"feed" filter in the tank. 

Pipe from refilling cap to tank.

Tank emptied and rinsed.

New glow plugs

Swapped High pressure diesel pump with a working engine.

Controlled diesel feed to pump.

swapped air filter box with mass airflow sensor from working engine. 

 

I have run diagnostics on the car and the fault codes that are showing are:

 

P1229 - fuel pump

P0100 - Mass air flow sensor

If i remove the plug on the SCV on the dieselpump the engine does not die, but it is in limp mode. It is still hard to start. ( for some reason the engine light doesn't light up - I would think it is supposed to since the car is in limp mode and the ECU does not communicate with SCV - am I right? )

 

I don't know what to do next. I have an extra car that had an working engine before I began dismantling it, so access to parts is no problem.

 

Can anyone help me???

hmm, You've already tried a lot of things!
What about the fuel pressure regulator?

It's attached to the pump, so it followed with the pump I swapped from the working engine.
