What could be wrong with my D4D T25? After driving it for 30 mins it just loses all its power(barely accelerates at 2-2.5k rpm) and starts driving all weird by weird I mean slow like it doesn't accelerate as it would on a cold engine and gears seem to just not shift right. No codes or lights on the dash... Could It be the EGR?

It is the EGR valve sticking.  Its been coked up.  It needs to be removed and cleaned. You will notice as soon as the engine reached warm/ normal operating temp the car will go into limp mode.  In limp mode you will not be able to push the car to drive above 25 to 35 miles/hr.
