Wheel making metal scrapping sound

I wanted to share my experience as nobody had  an answer to my problem and my mechanic said he had never seen anything like this before. 

I had a metal on metal scrapping sound, everybody I spoke with and every article I looked at said it is symptomatic of the brake guard catching the brake disc. 

Eventually after having check this and changed the front brake pads I had the rear brakes changed some months later as the noise didn’t go away when the fronts were changed. The mechanic showed me the old pads, the ones on the rear passenger side were worn at an angle which he said he had never seen or heard of before. He also said the brake pad which is located in position by a raised cylindrical shaped pin was not seated in its housing, and it appeared to  have  jumped off it/dislodged. 

I have had this car from new (2009) and done 150k miles and never had the rear brake pads replaced so this must have been like this when it left the factory. 

I hope this helps other, as the rear brake pads cannot be clearly seen simply by removing the wheel.

I had the same on my Avensis last year. It was due to the piston cut out not being vertical, so the pin on the back of the brake pad is not within the cut out. This causes the brake pad to wear at an angle or piston pushed to one side. The inner pads were badly worn, while the out pads were fine for many years. I had to replace the pads. Done properly this time by myself.
