HVAC - Blend door actuator problem

Hi, i brought my 2004 Toyota Corolla D4D earlier this year and it’s just come to my attention that AC is only blowing cold. 

After some poking around i found that the blend door actuator is damaged and it looks like somebody has attempted to repair in in the past. 

I found it disconnected and held together with zip ties and some screws missing and much to my disappointment the arm which operates the flaps has sheered completely off and missing.

Im looking online for examples of how the whole thing goes back together so i can purchase a new blend door actuator online and know it’s going to fit.

Does anybody have a diagram for the HVAC system? i can see where the actuator mounts but i can’t seem to figure out where or what they  actuator should be operating. in my opinion there is a part missing. lo

Is your car a LHD or RHD ? Hatch, sedan or Verso ? does the flap actually move ? or did the flap seize and cause the arm to break ?

It’s a LHD hatch back

I can’t tell you how it’s broken however it seems to have broken in two places in my opinion. 

 

once at the motor and the other break is where the arm attaches. i’ve been surfing the web and i think this is how i think it should attach i believe 

 

Although it’s a different model of car the principle looks the same

Is your car a Japanese or UK build one ? Jap cars VIN Starts with JT ?
