Oil Cooler - 2.2 Diesel - Oil in Coolant

Hello, is there a guide or diagrams to aid with changing the Oil Cooler on the 2.2 diesel engine? ( Avensis 2006 - 148bhp). There is Oil in the Coolant & I am guessing this is the most logical step. ( The Oil filler cap is reasonably clean) Thx Bryan
