In general, has the gearbox oil been checked? Also is the clutch consistent and not need pumping? What engine has the car got, because diesels normal have dual mass flywheel (DMF). DMF in Toyotas are generally reliable, but age, wear and tear can catch up. When I stop and need to engage reverse, sometimes I have pause and do a double de clutch to get a smooth reverse selection, if having a little trouble. This only happens once in a while.
