Does anyone know what the cause could be?

The most common reason for P2002 is a clogged DPF. This can happen due to frequent short trips, not reaching high enough engine temperatures for efficient regeneration, or simply reaching the end of the filter's lifespan

Faulty sensors monitoring the DPF pressure or differential pressure can also trigger the code, even if the filter itself isn't clogged

Leaks in the exhaust system before the DPF can disrupt the pressure readings and trigger the code. Hope this helps😄

As said above the most likely cause is a blocked DPF.  Try going for a drive on the motorway, keeping the engine above 2000 RPM for at least 30 minutes.  This will cause the DPF to attempt a regeneration, which if successful, should clear the code.

You can't tell when it is completing a regen.  It is just getting the DPF up to a high tempreature to burn off the soot.  I know on some cars like VW, the fan sometimes stays running after shutting the car off to cool the DPF.  I have never heard the Avensis doing this however.
