I have avensis 2012 d4d 2 l estate. 

When I'm Accelerating and reaching 2000 rpm car starting to jerck or like a miss firing and engine shaking so im afraid to damage engine,does anyone had similar problems and what'sthesolution?

is it a 2.0d4d  by chance id it the injectors need cleaned

Definitely could be injector related.  As a simple step, I would try running some fuel system cleaner, and see if this helps things.  I would recommend Liqui Moly 5156.  Have you serviced the car recently, including a new fuel filter?

Car been serviced every 10k km. No codes for injectors only usual parking brake and vsr system but that's doesn't affect. It's more miss fire in 4th 5th and 6th when reaching 2000 rpms.
