Avensis D4D engine sooting

Hey! 

I have some issues with my Toyota and hopefully someone can give me som pointer.

Last summer i had do change all four injectors because of leakage and they where programmed.

The new injectors where tested and everything were fine. 

Short while after that it startet to smoke like hell from the exhaust, all black, and a huge black smoke cloud when acceleration.

Took out the intakemanifold and it was all clogged and cleaned the intake, EGR valve and the throttle and put it all together.

Still sooting.

I also had a diesel leak from the return hose down to the engine wiring harnes down to the O2 sensor. 

I cleaned it all up but the O2 sensor voltage is still fixed at 0V even though the resistanse was ok (dont remember how many ohms) and the hose are mounted with new clamps.

I have no fault codes and i really dont know where the f**k I can look now.

Anyone have any experience with similar issue and have any tips to where to look?

Because im kind of out of ideas 🥲
