Warranty is confusing

The service history doesn't have to be a full Toyota service history - extract from Toyota Mag:

 "Lapsed and new customers who have previously chosen to have their vehicle serviced outside the Toyota network can also take advantage of the warranty. There is no requirement for a vehicle to have a history of servicing at an official centre in order to qualify for the warranty; it will be applied from the date the vehicle undergoes a qualifying service at an approved Toyota centre."

However, Relax doesn't cover damage or defects as a result of repairs not performed by an Authorised Toyota Repairer.

Can you take a 6 year old Toyota that had the first 3 services done then nothing in year 4 and 5 into the dealer and they will service it and give you the relax warranty?

On the face of it, yes - but I believe any existing defects highlighted by the dealer service will need rectifying at your expense.

suggests, strongly, that it must have been serviced correctly even if not by Toyota agents.

Implies it didn't get those services, or there is no recorded evidence of them.

My reply was correct - on the face of it the car could be covered under Relax.

However, existing faults won't be covered and would need to be repaired first at the owners cost prior to Relax being generated.

There are some members here that had been reporting warranty work rejected by some dealers even though they had all services correct. 
Any warranty  job it’s strictly individual case and depending on car history, ownership, use and also the dealer. Sometimes what is on paper it is not the same in reality.
