I always put the reasons for leaving are to start my new job etc. (pretty obvious really!). Whatever you do don't lie about your reasons. If you didn't like the job, say so and just say you quit to give yourself a better chance of finding a new job more suitable to yourself.

As for reasons to want to leave, as has been said, put the reasons. If it's that the job doesn't match your qualifications then say so. If you've "outgrown your current role" or are seeking something more challenging then say so.

Failing all else, ask someone to help you go over the application, they'll also help to point out any mistakes and hopefully suggest possible improvements.
