Check the ecu for any fault codes stored in there to see if it narrows it down a bit.

Plugs??

HT Leads??

Lamba Sensor??

Clogged air filter??

A simple oil change may correct the problem. The MOT garage should be able to give advice on this. Mine hadn't been serviced for 9 months when it was MOT'd and almost failed because of the state of the oil which affected emissions.

Does the MOT failure show what it failed on gaswise? My 3SFE failed on emmssions and it appears to have been the lambda sensor. You can get universal ones for the 3S-FE for £46 (requires fiddly soldering) and direct fit for about £10 more from Partco (Unipart). I fitted the sensor myself and it passed second time around. There was no service light on or anything to show a fault to begin with.
