Noise From Gearbox, Model 98

I have an Avensis with the 4A-FE engine. It has 210,000 klm on the clock. during the last 40,000 klm i keep hearing a high frequency noise from the gear box, with 3rd 4th and 5th gear, which is really irritating when i accelerate in a specific range of rpms and when cruising at 120klm with fifth.

In the past the local Mr T has changed twice a set of bearings under warranty (thats what i was told, however i doubt if they ve done sth...)

The noise was not easily recognised when they ve done the second service, so i couldnt ask for sth more then...

Any ideas what can it be?
