Avensis Gls Estate - Juddering When Going On/off Power

As per title really. When going through the gears, it seems to judder. I can't do smooth gear changes, as it judders between the changes. It's more pronounced in the lower ranges. Also when transitioning from gas to coasting (when approaching a junction or similar), it judders as it goes off power. My first thought is engine mountings worn out and needing replacement. Is this easy and cheap to do? It's an S-reg 3sfe engine, done about 80K -odd miles. Any advice?
