Yaris Over Revving

I am new to this forum, but keen to have some advice.

When driving up an incline my Yaris D4-D began over revving and seemed like it was taking off for the moon! This happened once before about 6 months ago under similar circumstances. Fortunately I was able to pull over and switch off, and after a short while re started and drove home up quite steep hills without further trouble.

I am concerned that this might happen in less favourable circumstances, and would be interested to have anyone's comments or suggestions as to what might have caused this.
